I am looking to get a custom made mouthguard. Any recommendations?

Yes! www.opro.com
I have one of these and it's great. It was expensive, about £40 but that's cos it's custom colours too. If you have a plain one I think you can get it for around £30. They send you a mould with a DVD of instructions, you make the mould, post it back and then they make it into a gum shield that fits perfectly.
I can speak pretty well with mine in and it's comfortable. I also got kicked hard in the jaw with it in just after I got it so I can say it works too! Laughing
Good luck!

Night Guards (Anti Bruxism Guards)

The soft night guard is made from a flexible material; this comes in two thicknesses, 2mm Night Guard or 3mm Night Guard. This type of guard is recommended for moderate to heavy grinding. This soft material in the guard cushions your teeth when you are grinding or clinching.
